BPW requires an original birth certificate with a raised seal. We must keep this birth certificate in our books all season long. Also, a physical signed by your doctor after Jan 1 2021 is also mandatory. Lastly, a copy of your child's end of the year 20/21 report card. If your child is entering kindergarten this year, this is not required.
If your son or daughter are registered for Middle school football ( grades 7 and 8 ) then an original birth certificate is not required and a copy is acceptable.
If you have already sent us this paperwork, thank you 🙂. If your child was in the program in the past, then we should have your child’s birth certificate.
Paperwork drop off (bring birth cert, physical and report card) will be this sunday July 18th from 5-8 pm. It will be at 5 Peach Orchard road. Please park across the street at MSMS and walk over.
If you are unable to make it (which I know is difficult during the summer months), You will need to email a copy of the physical and report card (if applicable) to
[email protected] and please drop off the original birth certificate in an envelope in my mailbox ( 5 Peach Orchard road) no later then Monday July 19th 2021.
